What’s Included in a Discharge Plan?
No matter which program you started in or paused, our team creates a discharge plan tailored just for you. If you enter our program and only stay through the detox phase, we help you plan what comes next after detox is over. This applies to every stage of treatment.
What is included in the discharge plan:
- What’s next: We help you plan for what comes next, including finding sober living facilities, Alcoholics Anonymous meetings, and Narcotics Anonymous meetings near you.
- Continued Treatment: We get you connected with mental health professionals and other outpatient services in the area to keep your recovery progress moving after initial treatment. We also help you find a sponsor and keep in contact with us for follow-ups.
- Alumni Program: Our alumni program is thriving, with daily chats, social media groups, and monthly events, so you can keep in touch with peers who have undergone the same treatment as you and are thriving in recovery
- Follow-up: We make it our mission to keep up with how you are doing. We make sure to follow up with you monthly after your treatment has ended. In addition, we are available 24/7 to help you with anything you may need on your journey.
The Client’s Role in Planning
We never want your discharge plans to come as a surprise, so we include you from the very beginning. All of your input, including your preferences for doctors and mental health specialists, is taken into account during discharge planning. We always take into account what you need to fit recovery plans into your home, work, and personal life, and we never just tell you what you are going to do.
Active Connections Before Leaving Treatment
Before you leave treatment, our case management team will help you put the right support in place so you feel prepared for life afterward. We take all the necessary steps, from setting up appointments and reaching out to your primary care physician to contacting your insurance to find in-network mental health providers.
Support for Clients Who Leave Early
Even if you decide to leave treatment early, we continue to support your recovery journey. We prepare a thorough discharge plan that includes scheduled appointments with mental health providers, information about available treatments, and education on what comes next.
No matter when you stop treatment, we will reach out to you for follow-ups on how you are doing and where your recovery journey is headed.
Planning for Long-Term Recovery
The power of connection is important for lasting recovery. That is why our alumni program is deeply integrated into our discharge planning. This program promotes a deep connection between you, our staff, and your fellow alumni. Through monthly meetings, constant communication, and peer support, we ensure you know you never have to walk your recovery path alone.
Our alumni coordinator, Brian Levenson, makes it his personal mission to reach out to all alumni each month to ensure they feel comfortable and accountable during their transition back to the real world. Brian is open to daily communication with alumni to support them whenever they need it, until they are comfortable on their own two feet.
